The Social Worker (MSW) at Hackensack Meridian Health Delivers Mental Health Services to patients including the assessment and ongoing treatment of Mental Health conditions.

Responsibilities

  • Performs clinical assessment of patients in the emergency room, inpatient units or at the off-site PMC Counseling Center for treatment needs and appropriateness of services atPMC, including referral to psychiatric hospitals, substance abuse services, community based mental health services, homecare services and/or other social services as appropriate.
  • Utilize knowledge of techniques and regulations appropriate to the ages and conditions of patients served. Communicate clearly and respectfully with patients and respond promptly to patient inquiries and requests.
  • Assess the patient upon telephone or during an in person intake sessions for treatment needs and appropriateness for services at PMC. Telephone assessment will be conducted on Crisis Hotline calls, as well as with telephone callers requesting out-patient mental health services.   • Make all necessary arrangements with and communicate all necessary information to these providers to meet the patient's needs as determined through assessment.
  • Deliver services as needed through scheduled psychotherapy sessions, and/or follow-up crisis stabilization work at PMC, whether on-site or at the off-site. Utilize knowledge of techniques and regulations appropriate to the ages and conditions of patients served. Communicates clearly and respectfully with patients and respond promptly to patients inquiries and requests.
  • Identify and intervene appropriately as needed to crisis situations, including but not limited to patients' threats of harm himself and others.
  • Document, on a daily basis, all relevant patient and collateral contacts.
  • Attend and participate in departmental staff meetings, whether on-site or off-site at the PMC Counseling Center.
  • Maintains social work licensure in good standing in the State of New Jersey including obtaining mandatory annual continuing education, with documentation of satisfactory attendance.
  • Provide daily charge tickets for all patient visits on a daily basis. Coordinates as needed with finance staff to provide accurate and complete information regarding patient accounts and billing.
  • Ensures delivery of excellent customer service resulting in positive and improving patient satisfaction scores for department and Medical Center.
  • Participates in Continual Quality Improvement efforts through participation on departmental and interdepartmental CQI
  • Organize and conduct therapeutic patient groups as needed.
  • Utilize knowledge of techniques and regulations appropriate to the ages of patients served.
  • Provide clinical supervision of social work gradute interns as required.
  • Updates knowledge of regulations and techniques appropriate to ages of patients served by participation in staff training and educational programs.
  • Maintains membership in the National Associate of Social Workers.
  • Performs other duties as required

Qualifications

EDUCATION:

  • Masters degree in Social Work
  • NJ Social Work license required.
  • LSW minimum.
