Trinity Health Of New England is looking for a Social Worker to join our Multiple Sclerosis Center Team. We are a member of Trinity Health Of New England family bringing compassionate care to our communities.
This position provides social work services to patients and families at Mandell MS Center of Trinity Health Of New England. The social worker is responsible for preforming psychosocial assessment, counseling, interventions and continuum-based interventions, education, and advocacy. The ideal candidate will provide a compassionate leadership role in developing and providing counseling and medical social work services to patients and families of patients who patients of the MS Center through the region.

Work Hours/Shift:

  • Full-Time

You Will Be Responsible For:

  • Exceptional Patient Care: Performs assessments, provides support to patients and families and is a patient advocate to assure appropriate cares is received.
  • Excellent Communicator: Effectively communicates with patients, families and the integrated clinical team to ensure everyone understands the needs of the patient and family
  • Care Management: Collaborates with Case Managers to make appropriate and timely referrals utilizing appropriate community resources to ensure continuity of care for patient and family (e.g. home care services, hospice, nursing homes, etc.).
  • Active Teamwork: Collaborates with the team, patient's physicians, therapists, nurses, case managers, integrative medicine, and spiritual care personnel to revise the care plan as necessary.


  • Education: Master's Degree Required
  • License: Valid social work license from the State of CT, minimally at the MSW level. LCSW Preferred
  • Experience: three (3) years clinical experience in medical social work.
  • Experience working with patients with multiple sclerosis and complex neurological diagnoses preferred.
