GENERAL SUMMARY : The Social Worker evaluates timeliness and appropriateness of care for members with dementia and their care partners and coordinates transition planning for changes in levels of needed care. The goal is to restore patients to optimum health and to ease social adjustments. Provides expeditious member memory and cognition assessments and collaborates with medical providers in determining the appropriate disposition and treatment for members and other vulnerable populations.

EDUCATION AND EXPERIENCE REQUIRED :

1. Licensed to practice as a Social Worker in the District of Columbia. Master's Degree in Social Work or Sociology, Gerontology, Special Education, Rehabilitation Counseling or Psychology from an accredited school.
2. Minimum of 2-5 years of social work experience in a health care setting along with one or more years' experience Case Management performing discharge planning from the inpatient or skilled facility setting.
3. Current CPR Certification required.
